Evidence - informed, collaborative care

Before requesting an appointment, please review the following:

• Please ensure you have an active primary care physician (PCP).

  Care is coordinated with your medical team for labs, monitoring, and broader health oversight.

• Prepare a brief summary (3–5 sentences) of your main concerns.

• Prepare a current list of medications and supplements.

• Bring any past evaluations to your first appointment if you have them.

• Appointments begin and end on time.

• Communication outside appointments is limited to brief administrative matters.

  The portal is not monitored continuously and is not for urgent concerns.

• Controlled substances are generally not prescribed.

  Rare exceptions are determined case-by-case with medical coordination.

  Safe prescribing requires accurate information about substance use.

  If concerns arise, additional evaluation (such as a urine drug screen) may be recommended.

• In safety situations, involving a trusted family member may be encouraged.
